Passport and Citizenship Services
All passport applications and Consular Reports of Birth Abroad (CRBA) are accepted by appointment only. To make an appointment call +48 61 851 8516. Applications are accepted in Poznan and then transmitted via courier service to the U.S. Embassy in Warsaw for processing.
Notarial services are performed by appointment only. To make an appointment call +48 61 851 8516. If the document to be notarized specifically requests witnesses, person requesting the service must arrange for witnesses to accompany him to the appointment. The Consular Agent cannot sign as a witness.
The customer can pay for the services in cash in local currency PLN only or by credit card. We accept Visa, Master Card, Discover, American Express and Diner’s Card.
U.S. Visa Services
